Getting Started

Read About Us

Please verify your insurance coverage and review our office policies. In order to establish care, prospective patients must carry an insurance coverage within our network and be receiving vaccines in accordance to state guidelines.

2.

Call Our Office

Following the birth of your child, call our office to speak with a member of our front staff and schedule your first appointment. Following discharge from the hospital, it is typically advised that newborns be seen by their primary care physician within 24 to 48 hours.

Complete Your Paperwork

Prior to your arrival for your Newborn appointment, complete the New Patient paperwork. This will be texted to you by our front staff or you can access it with the link above

Come On In

We look forward to meeting you and your new addition in the future! Come early to give yourself time unloading the car. If your child was born at a facility outside of the Corewell Health network, please bring your discharge papers to this visit.

Complete Your Paperwork

Before scheduling an appointment, you will need to complete your New Patient paperwork. This gives you time to review our policies and gather information from your previous provider. Use the link above to access the paperwork or call our front desk to receive it via SMS.

Change Your PCP

For Medicaid and most HMO coverages, you will need to change your primary care physician in order for your visits to be covered. You can select any provider within our practice as your PCP. Being a group practice, you can schedule visits with any provider here, not just your PCP.

Schedule

Following the completion and our review of your paperwork, a member from our office staff will call you to set up your first appointment at PHC. We will ask the date of their last visit with their previous physician so please have that handy. Records from previous providers will also be requested prior to your appointment.

4.

Come On In

We look forward to meeting you! Have your insurance card and a photo ID ready at check in. Notify our front staff if there are any forms you need completed during your physical.
